Wound Care Clinical Coordinator Job Summary

The Clinical Coordinator-Wound Care is responsible for performing assessment, discharge planning, resource utilization, and program planning to meet the needs of all wound care patients. This role requires a strong understanding of wound care principles and practices, as well as excellent communication and interpersonal skills.

Key Responsibilities:
  • Performs therapeutic modalities with an understanding of pathophysiology and indication for therapy.
  • Prepares, evaluates, and instructs patients in their wound management plan.
  • Charts information in the appropriate record in detail.
  • Provides safe administration of hyperbaric oxygen.

Requirements:
  • Associates Degree and a minimum experience of 3 years job-related experience.
  • Wound care or home care experience.
  • CT State RN license, BLS required.

Preferred Qualifications:
  • CWS or WOCN certification in wound care preferable.
  • Documented experience in wound care and hyperbaric medicine.
