Job Summary:

The Case Manager will provide support to patients and families, and technical or caseload support to the care management team. This role will involve meeting with patients in their homes, communities, or clinics, and assisting with skill building and achieving small steps toward goals they have set.

Responsibilities:

  • Provide case management services to patients and families, including assessment, planning, and coordination of care.
  • Meet with patients in their homes, communities, or clinics to provide support and guidance.
  • Assist patients with skill building and achieving small steps toward goals they have set.
  • Participate in groups such as Living Well with Chronic Conditions, or Chronic Disease Self-Management Program.
  • Advocate for patients with chronic conditions, and provide emotional support and guidance.
  • Handle all patient information and patient activities in a confidential manner.
  • Demonstrate professionalism and appropriate boundaries in all interactions.

Requirements:

  • High School Graduate, GED, or equivalent.
  • One year paid or volunteer experience providing medical, social service, or behavioral health case management or care coordination.
  • Background in community outreach or case management is preferred.
  • Must have attended or plan to attend shortly after hire: WA State Community Health Worker Training and/or Chronic Disease Self-Management training.
  • Must have and maintain a CPR certification or obtain it within the initial probationary period.
  • Must have a valid driver's license and proof of insurance.
  • Must have the ability to write routine reports and correspondence and ability to speak effectively before groups of customers or employees of the organization.
  • Bilingual in English/Spanish is preferred, but not required.
