Job Title: Pathways Community Health Worker

Sea Mar Community Health Centers is a community-based organization committed to providing quality, comprehensive health, human, housing, educational, and cultural services to diverse communities. We are seeking a highly motivated and culturally connected individual to join our Care Management team in Everett, WA.

Job Summary:

This full-time position involves working closely with clients, community organizations, and other departments to serve as a link between underserved communities and existing community resources. The successful candidate will have a background in community outreach or case management and knowledge of community resources. They will assist clients in overcoming barriers to health, social services, education, and employment through home visiting and other means.

Responsibilities:
  • Be a patient advocate and conduct standardized patient in-take and risk assessments.
  • Establish a trusting relationship with the target population, referred clients, and community partners.
  • Identify and assist clients with client-identified health and social service goals as appropriate.
  • Assist in the development of strategies to address or eliminate barriers to care.
  • Work collaboratively with the Program Manager, PCP, and other assigned personnel to ensure that all relevant client needs are identified and addressed.
  • Participate in ongoing assessment of the client and family needs to determine if expected outcomes are reached and to ensure implementation and/or adjustment of the service plan.
Requirements:
  • High School Diploma or Equivalent required.
  • Community outreach or case management experience preferred.
  • Experience working with people from various backgrounds required.
  • Experience working with individuals with substance use disorders, homelessness, chronic mental illness, or people in crisis preferred.
  • Prefer one year paid or volunteer experience providing medical, social service, or behavioral health case management or care coordination.
  • Bilingual English/Spanish is preferred, but not required.
