LPN - Behavioral Health Specialist

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Sunrise Services. As a LPN, you will play a vital role in providing high-quality care to our residents in a dynamic and supportive environment.
About the Role:
The LPN will be responsible for coordinating and delivering healthcare services to residents in our Enhanced Services Facility. This includes administering medications, documenting health records, and collaborating with a multidisciplinary team to ensure the best possible outcomes for our residents.
Key Responsibilities:
- Administer medications to residents and document their health records accurately.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Conduct basic behavioral observations and gather clinical data to inform care decisions.
- Communicate effectively with residents, families, and healthcare professionals to ensure seamless care transitions.
- Participate in quality improvement initiatives to enhance the overall quality of care.
Requirements:
- Current LPN licensure in the state of Washington.
- Associate's degree in nursing or equivalent education.
- Basic computer skills and proficiency in electronic medical records systems.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
