Job Summary

We are seeking a highly motivated and compassionate Case Manager and Community Outreach Coordinator to join our team at ICL. In this role, you will work closely with consumers living in independent apartments in the community, assisting them in developing and maintaining viable living, working, and social situations.

Your primary responsibility will be to provide supportive services on a schedule established by the program, helping consumers to modify attitudes and patterns of behavior by increasing their understanding of self and others. You will also secure information regarding medical, psychological, and social factors contributing to the individual's situation, evaluating these factors and completing consumer treatment plans as needed.

The ideal candidate will possess excellent communication skills, both written and verbal, and be able to work independently under general supervision. A strong background in human services, coupled with a commitment to equity, diversity, inclusion, and belonging, is essential for success in this role.

This position offers an estimated annual salary of $60,000-$80,000, depending on experience, and comes with a comprehensive benefits package, including health insurance, paid time off, and professional development opportunities.
