About The Job
We are seeking a skilled Social Worker to join our team in supporting adults with serious and persistent mental illness to live independently in their communities. This role involves coordinating services, providing family counseling, and developing treatment plans.

Main Responsibilities:
  • Culturally competent and sensitive service provision.
  • Collaboration with multidisciplinary teams, including medical, psychiatric, and psychology staff.
  • Development of treatment plans based on assessments from professionals and direct care workers.

Requirements:
  • Master's degree in Social Work (MSW) and LMSW/LCSW with at least one year of relevant experience.
  • Ability to work with diverse populations and travel to visit residents in apartments or escort them in the community.
