Position: Case Manager for Transitional Housing Solutions

Job Overview:

This is an exempt, full-time role requiring 40 hours per week. The Case Manager for Transitional Housing may necessitate some evening hours and flexibility in scheduling. This position is tasked with delivering case management and advocacy services to adult clients and their dependents enrolled in our transitional housing program. Key responsibilities include offering direct services such as emergency shelter, crisis intervention, short-term counseling, life skills training, and housing stability assistance while adhering to the agency's confidentiality standards. Additional tasks may involve physical activities such as lifting, painting, cleaning, and maintenance of housing units. The role reports directly to the Director of Housing.

Key Responsibilities:

  1. Conduct client intakes and assessments.
  2. Provide counseling, life skills assistance, advocacy, and other necessary direct services.
  3. Facilitate life skills workshops and tenant meetings.
  4. Offer transportation support.
  5. Develop safety and goal plans with clients, assisting them in achieving and maintaining those objectives.
  6. Monitor and assess progress towards established goals.
  7. Document all client interactions thoroughly.
  8. Maintain and update client files as needed.
  9. Implement and uphold agency policies and procedures to ensure client safety.
  10. Participate in hotline and on-call rotations.
  11. Other duties as assigned.
Position Requirements:

Education/Certification:
  • Valid Driver's License and Auto Insurance - Required.
  • Bachelor's Degree - Required.
  • 40-Hour Training in Domestic Violence - Provided.
  • Successful completion of a background check - Required.
Experience:
  • 1-3 Years of Relevant Experience - Required.
  • Experience working with Domestic Violence Victims - Preferred.
Abilities:
  • Ability to sit or stand for extended periods - Required.
  • Capacity to lift a minimum of 50 pounds - Required.
  • Excellent written and verbal communication skills - Required.
  • Basic computer proficiency - Required.
  • Ability to comprehend and interpret written information - Required.
  • Competency in recognizing and responding effectively to trauma responses - Required.
  • Sound judgment and problem-solving skills - Required.
  • Team-oriented mindset - Required.
  • Ability to empower and communicate effectively with clients - Required.
  • Commitment to non-violence in familial and domestic settings - Required.
