About the Role

Job Summary

In this critical role, you will be responsible for developing and implementing individualized treatment plans that address the unique needs of each patient. As a key member of our interdisciplinary team, you will work collaboratively with our healthcare teams to ensure seamless communication and coordination of care.

Responsibilities
  • Assess patient health and identify potential health risks through comprehensive physical examinations and medical histories.
  • Order and interpret laboratory tests and other diagnostic procedures to inform treatment decisions and ensure effective patient care.
  • Develop and implement treatment plans, including prescribing medications and therapies as needed.
  • Counsel patients on healthy lifestyle choices and disease prevention, while also educating them on their treatment plans and any necessary follow-up care.
Requirements
  • Current licensure as a Registered Nurse (RN) in Texas or compact state
  • National certification as a Nurse Practitioner (NP)
  • Basic Life Support (BLS) certification
  • Ability to maintain accurate and detailed patient records, ensuring timely and seamless communication with colleagues and healthcare providers
