Advanced Practice Provider

**Key Responsibilities:**
- Develop and implement individualized care plans that address the physical, emotional, and social needs of patients.
- Collaborate with healthcare professionals to coordinate patient care and ensure seamless transitions between settings.
- Provide comprehensive wound care services, including debridement, dressing changes, and education on proper wound care techniques.
- Conduct regular assessments to evaluate patient progress and adjust care plans as needed.
**Requirements:**
- A Master's Degree in Science of Nursing or Physician Assistant Studies is required.
- A minimum of 3 years of advanced practice provider experience is preferred.
- Advanced Practice Registered Nurse License or Physician Assistant License in the State of Texas is required.
- Family Nurse Practitioner or Adult Gerontology or Acute Care Nurse Practitioner certification or Physician Assistant Certification from National Commission on Certification of Physician Assistants is required.
**Salary Range:** $120,000 - $150,000 per year, depending on experience.
