Medical Care Specialist for Complex Needs

Job SummaryWe are seeking a compassionate and skilled Nurse Practitioner (APRN) to join our team. As a Medical Care Specialist for Complex Needs, you will be responsible for providing evidence-based medical care to patients with chronic illnesses and injuries.
Key Responsibilities- Examine and treat patients with chronic and urgent episodic illnesses, minor injuries, accidents, and other health issues.
- Order and interpret laboratory and diagnostic testing as needed.
- Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain comprehensive plans of care.
- Provide referrals to other healthcare practitioners when necessary.
- Ensure patient medical records are kept up-to-date.
To be successful in this role, you must have:
- An active state Nurse Practitioners license.
- A current Nurse Practitioner National Board Certification.
- An active state DEA licensure.
- A Master's degree in Nursing (MSN).
- Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
- Strong interpersonal communication skills – verbal and written.
- Excellent time management skills with accountability for practitioner's daily workflow.
- Proficiency in medical instruments and equipment.
- A strong knowledge base of HIPAA and OSHA regulations.
- Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
- A licensed driver with valid automobile insurance.
