Role Summary:
The RN Case Manager delivers expert nursing care to hospice patients in their residences or care facilities. This role involves assessing the needs of patients and their families, as well as planning, implementing, and evaluating nursing care in line with physician directives and the interdisciplinary care plan.

The compensation for the RN Case Manager ranges from $33.49 to $44.58, with a sign-on bonus available. (New graduates may start at a lower salary.)
Work Schedule:
The RN Case Manager typically works Monday through Friday, from 8:00 a.m. to 5:00 p.m.
Key Responsibilities:
- Conducting nursing assessments and revising care plans as necessary;
- Delivering direct hospice nursing care to ensure patients maintain their highest level of comfort and quality of life, while regularly evaluating nursing care needs;
- Implementing appropriate interventions and monitoring their effectiveness;
- Providing education to patients and their family members;
- Coordinating hospice nursing services when external agency involvement is necessary;
- Ensuring the availability of essential medical equipment, supplies, and services;
- Completing Home Health Aide tasks as required and conducting supervisory visits for instruction, oversight, and evaluation; and
- Providing on-call coverage as needed or scheduled.
Qualifications:
- Graduation from an accredited nursing program and possession of a current Registered Nurse (RN) license in the State of Michigan;
- Preferred two years of recent clinical experience, although recent nursing graduates are welcome to apply. Experience in homecare or hospice is advantageous;
- Demonstrated clinical judgment; and
- Knowledge and/or experience in managing pain and symptoms associated with end-of-life care.
