Job SummaryWe are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team. As a Private Duty Nurse, you will provide high-quality patient care in the comfort of their own homes. You will work closely with our patients, their families, and our interdisciplinary team to develop and implement personalized care plans that meet their unique needs.
Responsibilities- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Conduct thorough assessments of patients' physical and emotional needs, and develop and implement care plans to address these needs.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and communication.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
