About the Role

We are seeking a highly skilled and compassionate Private Duty Nurse LVN to join our team at Quality Talent Group. As a key member of our care team, you will play a vital role in providing exceptional patient care and support to our clients in the comfort of their own homes.

Responsibilities
  • Develop and Implement Care Plans: Collaborate with our interdisciplinary team to create personalized care plans that meet the unique needs of each patient.
  • Provide Skilled Nursing Care: Deliver high-quality nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Communicate Effectively: Maintain open and transparent communication with patients, families, and healthcare providers to ensure seamless care coordination.
  • Document Patient Progress: Accurately and thoroughly document patient progress, including assessments, interventions, and outcomes.
  • Participate in Interdisciplinary Team Meetings: Collaborate with our team of healthcare professionals to discuss patient care plans, share best practices, and address any concerns.
Requirements
  • Current LVN License: Possess a current and unencumbered LVN license in the state of practice.
  • CPR Certification: Hold current CPR certification from a recognized certifying organization.
  • Experience in Home Health Care: Have a minimum of six months of experience in home health care, preferably in a pediatric or geriatric setting.
  • Strong Communication Skills: Possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
