Aveanna Healthcare is a leading provider of home health services to medically fragile children and adults. We are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team as a Private Duty Nurse.

Job Summary

The Private Duty Nurse will provide one-on-one care to patients in their homes, assisting with daily living activities and providing medical care as needed. This is a part-time/PRN position, with flexible scheduling to accommodate your availability.

Responsibilities
  • Implement and update the nursing care plan in collaboration with the patient's physician and healthcare team.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Assess and document patient condition, identifying changes and reporting to the supervisor and/or physician as needed.
  • Collaborate with the patient's family and caregivers to ensure seamless care and support.
  • Participate in ongoing education and training to maintain current knowledge and skills.
Requirements
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
  • One year of experience as a Licensed Nurse in a clinical care setting (preference for home health experience).
