Job Summary

We are seeking a highly motivated and compassionate Care Specialist to join our team. As a Care Specialist, you will play a vital role in delivering chronic care management to high-complexity patients in their homes and communities. Your primary responsibility will be to conduct direct outreach to patients, enroll them in our program, and coordinate care with our interdisciplinary team.

Key Responsibilities
  • Conduct direct outreach to patients via phone, in-person, mailings, and other strategies to enroll them in our program and collect key data about patients.
  • Meet patients in their homes or in the community to conduct needs assessments, set health goals, and coordinate care with our team.
  • Support patients in meeting their healthcare goals related to quality measures and work with our team to meet market and/or organizational goals.
  • Analyze patient data to determine patient needs or treatment goals and assess physical conditions to aid in diagnosis, treatment, and referrals.
  • Explain technical medical information to educate patients and cultivate relationships with primary care providers to ensure seamless care coordination.
  • Build rapport with patients using motivational interviewing techniques and conduct one-on-one extended in-person patient appointments.
  • Document each patient encounter with accuracy and precision and prepare reports as needed.
Requirements
  • At least 2 years of relevant work experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role.
  • High school graduate or GED required.
  • A valid driver's license and auto liability insurance.
  • Reliable transportation and ability to travel within assigned territory or other as needed.
  • Experience in Chronic Care Management model or experience with chronically ill/elderly patients.
  • Long-time resident of the community with good knowledge of local resources.
  • Ability to complete Upward Health's initial training program and ongoing educational requirements.
  • Technologically savvy with basic computer skills, including ability to type.
  • Multi-lingual capabilities preferred but not required.
