Job Summary

We are seeking an experienced Registered Nurse to join our Hospice team as a Home Care Specialist. This role requires a strong background in hospice care and the ability to provide high-quality patient care in a home setting.

Responsibilities
  • Conduct comprehensive assessments of patients and their families to determine their home care needs.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide education and support to patients and their families on disease management, symptom control, and end-of-life care.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Participate in on-call duties as defined by the on-call policy, including night home visits to provide emotional support and symptom management.
Requirements
  • Graduate of an accredited nursing school with one to two years of recent acute care experience in a healthcare facility.
  • Current state licensure and CPR certification.
  • A bachelor's degree is preferred, with one year of home health care experience.
  • Prior management experience is not mandatory; responsibilities include supervising home health aides.
  • Strong observational, verbal and written communication skills, problem-solving abilities, and basic math skills are required, along with nursing skills as per the competency checklist.
Qualifications
  • Current and unrestricted RN license
  • 1 year: Hospice Experience
  • Current BLS/CPR certification
  • Valid Driver's License
