Job Description

Nurse Case Managers play a vital role in ensuring seamless patient transitions and optimal care outcomes. If you possess at least one year of RN experience and have completed an accredited registered nurse program, we encourage you to apply.

To succeed in this position, candidates must demonstrate excellent communication skills, attention to detail, and the ability to work effectively in fast-paced environments.

Required Skills and Qualifications

  • Active RN license or willingness to obtain a Registered Nurse license in Hawaii
  • BLS certification and relevant Case Management/department-specific certifications
  • At least one year of Case Management experience within the last two years
  • Minimum of one year of RN experience
  • RN degree from an accredited registered nurse program
