About the Role

Navian Hawaii seeks a skilled Pediatric Nurse Case Manager to join our team in providing exceptional care to patients and their families. As a member of our interdisciplinary group, you will play a vital role in assessing, planning, implementing, and evaluating patient care plans.

Duties and Responsibilities

The successful candidate will have primary responsibility for providing care to patients and families on our Pediatric Hospice Program. Key responsibilities include:

  • Working collaboratively with physicians and other healthcare professionals to develop and implement patient care plans.
  • Providing holistic care that addresses the physical, emotional, and spiritual needs of patients and their caregivers.
  • Documenting all nursing and case management activities in accordance with hospice protocols and maintaining open communication with attending physicians.
Requirements and Qualifications

To be considered for this role, applicants must possess the following qualifications:

  • A current Hawaii license as a registered nurse (BSN preferred).
  • At least two years of clinical experience in hospice and/or home health nursing (pediatric experience preferred).
Benefits and Compensation

We offer a competitive salary range of $93,000 to $99,000 per year, along with a comprehensive benefits package that includes a consistent Monday-to-Friday schedule, flexible work environment, and opportunities for professional growth and development.
