We are seeking a compassionate and skilled Clinical Nurse Specialist in Hospice Care to join our team at Navian Hawaii.

Responsibilities
  • Patient Admissions: Ensure seamless transitions from hospital or transfer facilities, communicating effectively with patients and caregivers to address their needs and concerns.
  • Nurse Case Management: Provide personalized case management services to patients and families, leveraging your expertise to optimize their experience and outcomes.
  • General Responsibilities: Support Quality Improvement initiatives, engage in ongoing education and professional development, and maintain productivity standards while adapting to changing patient needs.
Requirements
  • A current Hawaii license as a registered nurse (BSN preferred)
  • At least one year of experience as a staff nurse in an acute hospital setting (hospice and/or home health nursing experience a plus)
