South East Alaska Regional Health Consortium (SEARHC) is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring that patients receive safe, timely, effective, efficient, equitable, and patient-centered care.
Key Responsibilities- Assess and Coordinate Care: Assist the care team in coordinating patient care, assessing self-management skills, and encouraging wellness and autonomy through support, training, and coordination of community and/or referred services.
- Support Care Coordination: Support care coordination within and outside of the SEARHC health system to ensure a consistent, effective, and supportive system of care.
- Communicate with Patients and Caregivers: Act as a communication portal between patients and their care team, communicating lab results, performing point-of-care testing and coaching when relevant, generating patient letters, and completing relevant forms as needed.
- Advocate for Patients: Advocate for patient and patient caregivers at the service-delivery level, empower patient decision-making and self-care, and address patient needs in a timely manner.
- Participate in Departmental Improvement Efforts: Participate in departmental and clinical improvement efforts, maintain a clean and safe working environment, and maintain compliance with annual competencies.
- Identify Community Resources: Identify community resources, foster partnerships, and utilize resources effectively.
- Education: Associate's degree or 2 years of college preferably in social work, social services, healthcare administration, business administration, or a related field.
- Experience: Minimum 2 years of healthcare or behavioral health experience. Internal candidates with at least two years of experience as a Patient Access Representative or similar role at SEARHC will be considered qualified for this role.
- Knowledge and Skills: Available services at SEARHC, other tribal health organizations in Alaska, and community; clinical process and the ability to apply this knowledge in the working environment; customer service principles; safety and infection control principles; assessment, anticipation of needs, and data collection; use of equipment such as computers and medical equipment; oral and written communications.
