At Optum, we're revolutionizing the way healthcare is delivered. Our Senior Community Care (SCC) program is a pioneering initiative that brings together a multidisciplinary team to provide comprehensive care to patients in their homes, nursing homes, and assisted living facilities.

Key Responsibilities:

  • Conduct thorough assessments to identify patients' needs and develop personalized care plans.
  • Manage patients through acute medical situations, exacerbations of chronic conditions, and short-term sub-acute rehabilitation episodes.
  • Ensure accurate and complete documentation, including ICD-10 conditions.
  • Collaborate with the interdisciplinary care team to provide seamless care coordination.
  • Conduct advanced illness and advanced care planning conversations with patients and their caregivers.
  • Provide education and counseling to patients and their families.

Requirements:

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Active and unrestricted license in the state where you reside or ability to obtain by start date.
  • Active Nurse Practitioner certification through a national board (e.g., American Academy of Nurse Practitioners or American Nurses Credentialing Center).
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Ability to gain a collaborative practice agreement, if applicable in your state.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Access to reliable transportation.

Preferred Qualifications:

  • 1+ years of clinical experience in practice, preferably in a long-term care setting.
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end-of-life discussions.
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records.
