  • Coordinate care with primary care physicians, specialists, pharmacies, and other medical services to ensure comprehensive and coordinated care.
  • Work collaboratively with a multi-disciplinary team to develop and implement individualized plans of care.
  • Direct and oversee the preparation of paperwork and coordination of appointments, including follow-up care as needed.
  • Monitor and ensure the safe and compliant administration of medications, including medication and treatment communication charts.
  • Participate in hospital discharge planning, hospice intake, and VNA supports within the community.
  • Verify that healthcare binders are organized and current, and ensure that healthcare records are accurately entered in HCSIS and kept up to date.
  • Facilitate health education and teaching to direct care staff and participate in other trainings and meetings as required.
  • Conduct regular site visits to review medications and discuss any issues or educational needs with staff.
  • Conduct medical audits to ensure compliance, safety, and well-being of all involved.
  • Travel to surrounding areas, including Malden, Reading, Saugus, Danvers, Stoneham, Melrose, and Peabody, as needed.

  • MA RN License (required)
  • Knowledge of case management systems, community service, and delivery systems for DD population (required)
  • Attention to detail and excellent time management skills (required)
  • Strong clinical communication skills (required)
  • Minimum of 1-year experience working with DD adults in long-term care or human service agency (preferred)
  • Experience with staff training and development (preferred)
