Job Summary: NAMI San Diego is seeking a highly motivated and compassionate Bilingual Homeless Outreach Worker to join our Mobile Navigation Team. The successful candidate will provide mobile, street, and field-based outreach services in the Central Region of San Diego County, working closely with homeless individuals to address barriers to housing, services, and healthcare needs.

Key Responsibilities:

  • Conduct mobile/street outreach to develop trusted relationships with homeless individuals, building rapport to assist with addressing barriers to housing, services, and healthcare needs.
  • Link and provide transportation to homeless individuals to the NAMI SD/IC Clubhouse, educating them on membership and the International Clubhouse model, and enrolling them as members when interested.
  • Complete necessary documentation to record member progress and meet with active individuals/members to review, evaluate, and support completion of their goals.
  • Conduct a VI-SPDAT (Vulnerability Index-Service Prioritization Decisions Assistance Tool) on all new members to connect them to proper interventions and services, entering results into HMIS in a timely manner.
  • Assist members with benefits establishment, obtaining birth certificates, Social Security cards, housing referrals, and health-related assistance, working collaboratively with local law enforcement and service providers to ensure a smooth transition from streets to interim housing.
  • Ensure and oversee mobile navigation vehicle maintenance, taking care of regular oil, gas fill-up, and mileage logs, and providing case management services and advocacy within the Clubhouse and community.
  • Record and maintain member profile/goal sheets within the first 30 days of enrollment, updating every 6 months or more frequently at member request, and updating emergency fact sheets annually or as needed.

Requirements:

  • Bachelor's degree in a human resource field and one year of experience working with individuals experiencing homelessness, mental illness, or substance use disorders, or with vulnerable populations, or equivalent work experience.
  • Required experience in homeless outreach and fluency in English and an additional language required in one of the other 8 threshold languages: Arabic, Chinese, Korean, Persian, Somali, Spanish, Filipino (Tagalog), or Vietnamese.
  • Preferred lived experience in SMI, SUD, or housing insecurity of self or direct support to an individual with SMI, SUD, or Housing Insecurity, and a satisfactory driving record.
  • Able to communicate effectively, verbally and in writing, and basic knowledge of computer software (Microsoft Office, Word, Excel) and web navigation.
  • Demonstrated ability to maintain confidentiality and reliable and consistent attendance.
