Job Summary

We are seeking a highly skilled RN Stroke Navigator to join our team at Providence Oregon Regional Shared Services. As a RN Stroke Navigator, you will be the first point of contact for patients before hospital discharge and will provide coordination between the patient, PCP, neurologist, and other specialists. This role requires strong communication and organizational skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Perform in-hospital early post-discharge screening for medication compliance, healthy lifestyle, depression, medical complications, and caregiver burden.
  • Provide coordination between the patient, PCP, neurologist, and other specialists to ensure seamless care.
  • Act as the patient's advocate and anticipate their needs.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Document patient information and progress in a timely and accurate manner.
Requirements
  • Bachelor's Degree in Nursing or obtained within 3 years.
  • Oregon Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association upon hire.
  • 5 years of related neurological clinical experience.
Preferred Qualifications
  • Master's Degree in Nursing.
  • NIHSS Certification upon hire.
  • Stroke Certified Registered Nurse upon hire.
  • American Association of Neuroscience Nursing membership and certification upon hire.
