Community Care Coordinator Role

We are seeking a skilled Community Care Coordinator to join our team at NYC Health Hospitals. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to provide coordinated care and support services.

Duties and Responsibilities
  • Collaborate with patients, families, and healthcare professionals to develop and implement comprehensive care plans.
  • Coordinate care services, including nursing, rehabilitation, and nutritional support.
  • Monitor patient progress, adjusting care plans as needed to ensure optimal outcomes.
Requirements
  • A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State.
  • Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
  • Two years of experience in a related field, preferably in home care or long-term care.
Salary and Benefits

The estimated salary range for this position is $90,000 - $115,000 per annum, depending on experience.
