Job Summary:
The Community Care Coordinator provides vital support to our residential program, ensuring the well-being and safety of our residents and their families.
Key Responsibilities:
- Maintains accurate daily records, including logbooks and shift reports.
- Conducts regular inspections of the facility, identifying areas for improvement.
- Welcome new families, provide orientation, and educate them on our house rules.
- Monitor residents' behavior, recognizing signs of distress or concern.
- Ensure common areas are clean, safe, and compliant with our regulations.
- Handle incidents according to established protocols.
- Update the daily census and provide critical information to relevant stakeholders.
- Exhibit exceptional telephone skills, transferring calls efficiently.
- Attend program and agency staff meetings, as well as training sessions.
- Visit multiple units as needed, reporting incidents to the Operations Manager.
Requirements:
- A High School Diploma or equivalent (GED) is the minimum educational requirement.
- A valid NYS Security Guard License is mandatory.
- Criminal background checks are required.
- NYS Mandate Reporter Certification is necessary.
- A valid Safety Coordinator (F80) certification is also required.
- A Driver's License is preferred.
Estimated Salary: $45,000 - $55,000 per year, depending on experience and qualifications.
