Case Manager/Client Care Coordinator

Volunteers of America-Greater New York (VOA-Greater New York) is a leading anti-poverty organization dedicated to ending homelessness in the New York area by 2050. We operate over 60 programs across New York City, Northern New Jersey, and Westchester County, providing emergency, transitional, and permanent housing services to individuals and families in need.

The Case Manager/Client Care Coordinator is a key provider of services within VOA-Greater New York's Case Management Model. This role involves working with clients to assess their strengths and needs, plan a course of action, link them to services, counsel them, and monitor progress. The Case Manager must ensure that clients receive appropriate, competent, and professional information and advice, referrals, and advocacy services to achieve successful outcomes.

This position requires a Bachelor's degree or 60 college credits and two years of relevant experience. The ideal candidate will have a demonstrated ability in verbal and written communication, including basic computer skills.

The Case Manager/Client Care Coordinator will be responsible for managing client intake and assessment, developing individual service plans, meeting with clients to assist them in obtaining needed services, and documenting case management activities. The role also involves providing aftercare to assigned clients who have been placed or discharged from the facility.
