Job Title: Care Coordinator

Job Summary:

The Care Coordinator provides critical support to HIV-positive clients living with complex medical conditions, severe mental illness, substance abuse, and long-term care needs. This role involves linking clients to a broad range of services essential to successfully living in the community.

Key Responsibilities:

  • Identify and engage clients from the NYS DOH list of eligible Brooklyn and Manhattan Medicaid beneficiaries.
  • Ensure clients attend medical appointments at outpatient clinics and comply with prescription regimens.
  • Develop and maintain clear service plans and progress notes, documenting service visits meticulously.
  • Manage a caseload of clients, providing at least one service per calendar month.
  • Coordinate ancillary care to ensure continuous, comprehensive care.
  • Collaborate with the Medical Home team, primary care and specialty doctors, and partners on a regular basis.
  • Initiate referrals to additional services, including home care, and notify support staff of referrals.
  • Assist clients in accessing transportation and research community resources to meet non-medical needs.
  • Work with the Medical Home team to ensure efficient use of inpatient resources and timely discharges.
  • Facilitate client care conferences for medical and peer review.
  • Collaborate with mental health and substance abuse care providers to ensure coordination of services.
  • Work with the ValueOptions Field Care Coordinator to provide and link clients to services that facilitate illness management.
  • Provide verbal and written illness management educational protocols.
  • Report serious incidents, allegations, incidents, or sensitive situations to supervisors.

Requirements:

  • B.A. degree in a related field.
  • 2 years of experience in providing direct services to mentally disabled clients or linking mentally disabled clients to services.

Preferred Qualifications:

  • Basic knowledge of mental illness and serious emotional disturbances.
  • Basic knowledge of treatment, rehabilitation, and community support programs.
  • Bilingual English/Spanish.
