Job Summary: We are seeking a highly skilled Medical Service Coordinator to join our team. As a key member of our nursing staff, you will be responsible for providing skilled nursing services and supervising in-home staff. You will work closely with our interdisciplinary team to develop and implement patient care plans, ensuring that our patients receive the best possible care.
Responsibilities:
- Provide orientation, inservice education, supervision and evaluation to in-home services staff as may be deemed necessary.
- Ensure services are within the parameters defined by the patient plan of care and agency policy and procedure.
- Completes patient visits as necessary and develops and updates the nursing plan of care and required documentation.
- Provides direct hands on nursing care to patients and families, as defined in the IDT plan of care.
- Assists with nursing care management and case finding visits as scheduled and as defined within the plan of care.
- Conducts initial assessments of new referrals.
- Implements the plan of care, including the provision of skilled nursing services as authorized by a physician.
- Provides appropriate input to the interdisciplinary team, including insight regarding patient and family needs and anticipated needs.
- Maintains liaisons, as may be assigned, with community referral sources, vendors and contracted service facilities/organizations, working diligently to ensure that a strong sense of collaboration and team building is evident.
- Prepares required reports and documentation in a timely manner and in accordance with agency defined parameters.
- Maintains an obligation to report wrongdoing/violation of agency policies, applicable federal, state and local laws, and regulations, pertaining to agency operations, to immediate supervisor or identified compliance officer.
- Adheres to and reflects organizational values in daily work.
- Completes all mandatory inservice education programs and completes any other additional inservice hours that are minimally required for the position.
Requirements:
- Current New York State nursing license and registration as a registered professional nurse.
- Effective verbal and written communication skills.
- Computer literacy and the ability to work effectively with electronic medical records.
- BLS certification.
- Acceptable driver's license and automobile available for use with current insurance.
- Availability by telephone.

Salary: $65,000 - $85,000 per year, depending on experience.
