**Job Description**

We are seeking an experienced and detail-oriented Care Coordination Specialist to join our team at Person Centered Services. In this role, you will provide critical administrative support to our Care Coordination department, ensuring the efficient delivery of services to individuals with intellectual and developmental disabilities.

About the Department

The Care Coordination department is a dynamic team dedicated to delivering high-quality support services to individuals with intellectual and developmental disabilities. Our team members are passionate about making a positive impact in the lives of those we serve.

Responsibilities

The Care Coordination Specialist will be responsible for:

  • Performing a range of administrative tasks, including data entry, filing, and record-keeping.
  • Providing exceptional customer service to internal and external stakeholders.
  • Responding to inquiries and escalating issues as necessary.
  • Assisting with the maintenance of department databases.
  • Creating and maintaining department forms.
  • Developing and maintaining department spreadsheets/trackers/reports.
  • Attending department meetings to take notes.
  • Coordinating meetings, room bookings, and maintaining shared drives/calendars.
  • Assisting with the submission of applications/referrals for services and benefits/entitlements.
  • Uploading and submitting documents to statewide databases/authorization systems.
  • Ensuring the integrity of departmental data and documentation.
  • Participating in ongoing training and professional development opportunities.

Requirements

To be successful in this role, you will require:

  • A High School Diploma/GED.
  • Excellent communication and organizational skills.
  • Ability to maintain confidentiality and handle sensitive information.
  • Strong computer skills, including Microsoft Office and Excel.
  • Reliable transportation and a willingness to travel within the service area.
