Position Overview:

Role Summary:

The Service Coordination Specialist plays a crucial role in leading discussions and documenting the identified requirements and strategies to address those needs. This position is accountable for preparing and submitting necessary documentation mandated by the Department of Aging and Disability Services to facilitate individuals in obtaining and sustaining their services and support systems. Additionally, the Specialist will oversee service provision, update documentation as individual circumstances evolve, and assist clients in preserving their benefits.

Key Responsibilities:

  • Lead and document meetings to assess individual needs and outline actionable steps.
  • Conduct quarterly evaluations of service delivery.
  • Prepare and submit required documentation to the Department of Aging and Disability Services to ensure continuity of services.
  • Support individuals in maintaining their benefits.
  • Ensure compliance with HCS guidelines for the procurement of adaptive aids.
  • Notify the Service Coordinator of any changes in individual needs and service delivery.
  • Collaborate effectively with all team members.
  • Maintain open lines of communication with individuals, their families, and team members.
  • Participate in the monthly on-call rotation for residential services.
  • Perform additional duties as assigned by the Area Director.

Qualifications:

  • High school diploma, GED, or competency test with three letters of recommendation.
  • Experience in providing services within the HCS program is preferred.
  • Ability to obtain and maintain CPR certification and Prevention training.
  • Must complete Prevention and Management of Aggressive Behavior (PMAB) training, demonstrating the physical skills necessary for effective intervention.
  • Possess a valid Texas State Driver's License.
  • Maintain current automobile insurance and the ability to travel as needed.

Compensation:
