Position Overview:

Role Responsibilities:

The Service Coordination Specialist plays a crucial role in leading discussions and documenting the identified requirements and strategies to address those needs. This position involves completing and submitting necessary documentation mandated by the Department of Aging and Disability Services to help individuals access and retain essential services and support. Additionally, the Specialist is responsible for overseeing service delivery, updating documentation as individual needs evolve, and assisting clients in maintaining their benefits.

Key Duties Include:

  • Lead and document meetings to assess individual needs and outline actionable steps.
  • Conduct quarterly evaluations of service delivery.
  • Prepare and submit required documentation to the Department of Aging and Disability Services to ensure ongoing support for individuals.
  • Assist clients in the maintenance of their benefits.
  • Ensure the procurement of adaptive aids complies with HCS guidelines.
  • Notify the Service Coordinator of any changes in individual needs and service delivery.
  • Collaborate effectively with all team members.
  • Maintain open lines of communication with clients, their families, and team members.
  • Participate in a monthly on-call rotation for residential services.
  • Perform additional tasks as assigned by the Area Director.

Essential Qualifications:

  • High school diploma, GED, or competency test accompanied by three letters of recommendation.
  • Experience in providing services within the HCS program is preferred.
  • Certification in CPR and Prevention is required.
  • Ability to complete training in Prevention and Management of Aggressive Behavior (PMAB).
    **This role requires the ability to perform physical skills necessary for PMAB, which can be both mentally and physically demanding. Candidates must demonstrate the capability to perform various physical tasks, including lifting and supporting individuals safely.**
  • A valid Texas State Driver's License is required.
  • Current automobile insurance and the ability to travel to clients as needed.

  • Ability to work independently while meeting community and HHSC standards with high-quality outcomes.
