Compensation: $60,000 - $65,000 Annually
Position Summary:
The Program Coordinator is responsible for managing the daily operations of the Dayspring Commons Apartment Building, which comprises 62 affordable housing units, including 37 units designated for individuals who have experienced homelessness and have significant mental health challenges. This role requires collaboration with various on-site personnel from different departments to develop effective problem-solving strategies and align operational approaches. Proof of COVID-19 vaccination is mandatory.
Key Responsibilities:
- Collaborate with Facilities and Security teams to ensure safe and compliant operation of all residential units.
- Conduct regular meetings with the director, service staff, security personnel, property management, and clients to maintain service quality and compliance.
- Screen potential clients and evaluate supporting documentation for all referrals to supportive housing.
- Manage the intake process and create comprehensive assessments for new participants.
- Establish systems to enhance the quality and efficiency of case management services.
- Ensure staff accurately identifies client needs, provides suitable referrals, and follows up to support clients in achieving their service plan objectives.
- Maintain thorough documentation of all assessments, service plans, and client interactions.
- Review all necessary case management documents and data collection tools in use.
- Intervene or coordinate interventions for clients in crisis, ensuring respect for client concerns.
- Supervise service staff, observing interactions with program participants and integrating feedback into supervision sessions.
- Identify areas for professional development among case management staff and provide necessary coaching and training.
- Deliver ongoing education and guidance to staff regarding professional interventions aligned with agency policies and evidence-based practices.
- Build partnerships and networks with community service providers.
- Maintain and update all required data information systems for the project.
- Ensure timely completion and submission of all required documentation and reports.
- Provide crisis intervention and stabilization as necessary, which may include after-hours or weekend follow-up for emergencies.
- Assess program needs and make informed recommendations.
- Perform related duties as assigned.
Qualifications:
Applicants must possess an LMSW with a minimum of three years of relevant experience, including supervisory responsibilities. LCSW and LMHC credentials are preferred. Fluency in Spanish is highly desirable. Candidates should be prepared to work some evenings and occasional weekends as required. Strong writing skills, including proficiency in spelling, grammar, and punctuation, along with excellent oral communication abilities, are essential. Experience in crisis intervention techniques and strength-based approaches is preferred. Candidates should demonstrate strong skills in assessment, direct practice, counseling, supervision, community outreach, group leadership, and program development. Proficiency in MS Word, Excel, PowerPoint, and database management is required. A valid NYS driver's license and access to a vehicle are necessary.
