Clinical Liaison for Hospice Services

About Alive Hospice
SUMMARY
The Hospice Clinical Liaison is a vital healthcare professional focused on hospital outreach, managing referrals, and enhancing admission rates.
KEY RESPONSIBILITIES
- Collaborate with clinical leadership to identify and cultivate potential referral sources and health-related groups, fostering professional relationships to promote hospice services and educate on the hospice benefit.
- Exhibit exceptional customer service and critical thinking skills consistently.
- Work with the medical community, patients, and families to facilitate eligible hospice admissions.
- Engage with referral partners within the hospital to identify and facilitate admissions for patients eligible for GIP level of care.
- Oversee patient discharge processes from the hospital and manage admissions to home, IPU, or long-term care facilities.
- Coordinate tracking of referral sources and assess the utilization of hospice services.
- Maintain relationships with contracted skilled nursing and long-term care facilities.
- Work independently with minimal supervision.
- Organize educational presentations and in-services.
- Demonstrate effective resource management while considering budget implications.
- Keep a detailed log of referral source expenditures.
- Document daily activities within the EMR and CRM systems.
- Conduct on-site assessments to identify needs and provide support to prospective patients and referral sources.
- Communicate effectively with physicians and healthcare providers regarding patient evaluations and care plans.
- Complete necessary admission documentation as required.
- Facilitate appropriate interventions during complex referral and admission processes.
- Perform additional duties as assigned.
No administrative supervisory responsibilities of agency staff.
CONTINUING EDUCATION
This position requires completion of 15 hours of continuing education annually, focusing on topics that enhance the ability to fulfill the responsibilities outlined above. Regulatory agencies may mandate additional hours for certain disciplines.
QUALIFICATIONS
To succeed in this role, candidates must demonstrate the ability to perform essential duties satisfactorily, meeting required competencies. Reasonable accommodations may be provided for individuals with disabilities.
EDUCATION and EXPERIENCE
Registered Nurse with a minimum of five years of experience in healthcare, preferably in hospice sales and marketing.
CERTIFICATES, LICENSES, REGISTRATIONS
Must possess a current Tennessee or compact RN license, along with a valid Tennessee driver's license and automobile insurance as mandated by state law.
