- Organize follow-up consultations, appointments, and support services in accordance with established service standards.
- Compile succinct patient summaries for the healthcare team and document recommendations in line with standardized care protocols.
- Design and implement patient education initiatives on subjects such as Hypertension, Breast Cancer Screenings, and Diabetes, contributing to projects that aim to enhance healthcare outcomes.
- Monitor patient progress throughout the care continuum, including during hospital admissions, and collaborate with inpatient care management teams.
- Prepare and present periodic reports on outcomes and performance improvement efforts.
- Comply with established productivity benchmarks.
- Engage in training and professional development opportunities to expand knowledge and improve practices.
- Foster positive relationships with key departments, including physicians, administrative staff, diagnostic teams, nursing staff, and community partners.
- Oversee detailed Quality Improvement Dashboards.
- Gather and record patient feedback and information within the Electronic Medical Record (EMR) system.
- Provide program updates or relevant anecdotes as necessary.
- Be open to traveling between various clinic locations if required.
- Perform additional duties as assigned.
- High school diploma or GED is required (Associate's degree is preferred).
- Certification as a Medical Assistant is preferred.
- Understanding of HEDIS measures and UDS is essential.
- 1-2 years of experience in Federally Qualified Health Centers (FQHCs) or community health settings.
- Completion of a Patient Navigator Training Collaborative or equivalent education and experience is beneficial.
- Experience with EPIC or similar electronic health record systems is preferred.
- Strong problem-solving, decision-making, and analytical skills are necessary.
- Typing proficiency of 35 WPM is required.
- Exceptional leadership, organizational, written, verbal communication, and interpersonal skills are essential.
- Ability to work independently and effectively lead teams.
- Strong presentation abilities.
- Adaptability to change and innovation is important.
- Capacity to collaborate effectively with team members.
- Aptitude for implementing both professional and community-based educational programs.
- Proficiency in Microsoft Office applications is required.
- Must provide proof of up-to-date COVID-19 vaccinations, including all recommended doses and booster shots when eligible.
