Medical Care Coordinator

We are seeking a highly motivated and detail-oriented Medical Care Coordinator to join our team at DHVAJ. As a Medical Care Coordinator, you will play a critical role in ensuring the delivery of high-quality patient care and improving health outcomes.
Key Responsibilities- Conduct comprehensive assessments of patients' medical, behavioral, and social needs to identify gaps in care.
- Develop and implement individualized care plans in collaboration with patients, caregivers, and healthcare providers.
- Provide education and support to patients and their families to enhance their knowledge and skills related to health and lifestyle management.
- Coordinate and modify care plans with patients, caregivers, PCPs, specialists, community resources, and other healthcare professionals as needed.
- Document all activities in the EHR and care management tracking system, following EHRPMC standards.
- Certified Medical Assistant (CMA) certification required.
- Ability to create custom comprehensive care plans for patients.
- Ability to incorporate both behavioral health and physical health management in an EHR/EMR system preferred.
- Excellent verbal and written communication and interpersonal skills required.
- Computer proficiency required.
- Ability to interact with patients, physicians, and other healthcare professionals in a professional manner required.
