Key Responsibilities:
  • Ensure timely scheduling, completion, and submission of MDS assessments.
  • Oversee the completion of PPS and OBRA assessments and RAPS.
  • Coordinate the gathering of information for care plans and assessments.
  • Monitor and audit medication administration records and physician orders.
  • Evaluate prospective residents in collaboration with the healthcare team.
  • Utilize data reports for quality improvement initiatives.
  • Facilitate patient and family care conferences.
  • Participate in interdisciplinary committees.
  • Supervise the input and transmission of MDS assessments to the state.
  • Manage the care planning process effectively.
  • Perform nursing duties as necessary.
  • Complete additional tasks as assigned.
Qualifications:
  • Graduate of an accredited nursing program; RN preferred.
  • Current nursing license in the state of employment.
  • Valid CPR certification.
  • Preferred: One year of nursing experience.
Skills and Abilities:
  • Effective communication skills with residents, families, and team members.
  • Strong organizational and prioritization abilities.
  • Commitment to maintaining confidentiality.
  • Detail-oriented with a focus on accuracy.
  • Knowledge of RAI requirements.
  • Ability to work collaboratively within a team environment.
  • Quick decision-making skills in emergency situations.
