The MDS Licensed Practical Nurse is a key member of our healthcare team, responsible for ensuring the timely and accurate completion of Minimum Data Set (MDS) assessments and care plans.
Key Responsibilities- Monitor and guide the completion of PPS and OBRA assessments and RAPS.
- Primary UR Coordinator, overseeing the gathering of information for assessments and care plans.
- Monitor and audit MAR's, TAR's, flow sheets, and physician orders to ensure accuracy and compliance.
- Assess prospective residents in conjunction with other team members to ensure a comprehensive understanding of their needs.
- Obtain data warehoused reports and QI/QM reports to inform care planning decisions.
- Schedule and facilitate in-patient/family care conferences to ensure resident-centered care.
- Participate on various committees to contribute to the development of policies and procedures.
- Input or supervise the input of MDS assessments, transmissions to the state, and obtaining receipt of validation.
- Manage the Care Planning process to ensure resident-centered care.
- Perform the functions of a nurse when required, providing direct care to residents.
- Graduate of an accredited School of Nursing, RN preferred.
- Valid nursing license in the state employed.
- Valid CPR certification.
- One year of nursing experience preferred.
