Patient Care Associate

Qualifications:
- Mandatory: Candidates must be at least 21 years old.
- Mandatory: A high school diploma or equivalent is required.
- Mandatory: Flexibility to work any shift or unit is essential.
- Preferred: At least one year of experience with the specific demographic served by the facility.
Key Responsibilities:
- Ensure the safety and well-being of patients while fostering a positive, supportive, and structured atmosphere.
- Conduct regular safety checks and maintain supervision at 15-minute intervals as outlined in special precautions or individualized supervision protocols.
- Accurately document clinical information in the patient's medical records in a timely manner.
- Assist in creating a safe, secure, and comfortable environment for patients, their families, and staff members.
- Engage with patients routinely, observe their behaviors, and communicate significant findings to the nursing team.
- Support patients with daily living activities, including toileting, bathing, dressing, grooming, oral hygiene, meals, hydration, and changing bed linens.
- May be responsible for obtaining and documenting patients' vital signs, height, and weight as assigned.
- Facilitate educational and rehabilitative group sessions covering various topics such as social skills, coping strategies, anger management, and independent living skills.
- Encourage patient participation in activities aimed at achieving treatment objectives.
- Complete and maintain necessary documentation, assisting with follow-up and paperwork related to incidents and events within the facility.
- May provide or coordinate transportation for patients with appropriate staff members.
Compensation: Based on experience.
