We are in search of a Pharmacist to become a part of our Ambulatory Pharmacy team. This role is distinctive as it involves joining a newly established, interdisciplinary team that provides support to our Primary Care South Nampa Clinic through virtual means from a clinic in Boise. In this capacity, you will be a vital member of an outpatient healthcare team dedicated to enhancing quality and patient outcomes through effective medication management.

Key Responsibilities:

  • Act as a crucial member of our integrated healthcare services team, addressing medication requirements and fostering long-term relationships with patients.
  • Utilize consumer insights to influence the care environment positively.
  • Collaborate with a multidisciplinary team of healthcare professionals, including physicians, advanced practice providers, behavioral health experts, and care coordinators, while maintaining a flexible approach to meet patient needs and preferences.
  • Provide care to pediatric, adult, and senior patient demographics, managing new health issues, chronic conditions, preventive care, and non-emergency acute care.
  • Work alongside the pharmacy team to manage 340b medication referrals through pharmacotherapy and medication therapy management services.

Required Qualifications:

  • Graduation from an accredited pharmacy program.
  • Possession of a valid Idaho Pharmacist License.

Preferred Qualifications:

  • Experience in clinical management of chronic diseases within a primary care setting.
  • Completion of a PGY2 Ambulatory Care Residency or a PGY1 pharmacy residency with a focus on ambulatory care, or equivalent experience in ambulatory care.
  • Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP).
