ROLE DESCRIPTION
The Caseworker/Supports Coordinator plays a vital role in delivering essential services, primarily focusing on identifying, coordinating, and overseeing necessary services and supports for waiver participants.


Key Responsibilities:
1. **Service Identification:** Assist participants and their families in connecting with, arranging, and securing services outlined in an Individual Support Plan (ISP), which may include medical, social, educational, or other community services.
2. **Coordination of Services:** Develop and manage the ISP collaboratively with the participant, their family, ISP team members, and service providers.
3. **Ongoing Monitoring:** Maintain regular contact with participants and their families to ensure that services are delivered according to the participant's plan.
4. **Support in Decision-Making:** Provide information and guidance to help participants choose whether to direct their own services and offer assistance for those who opt for participant-directed services.


WORK SCHEDULE AND ADDITIONAL INFORMATION:
This is a full-time position with standard working hours from 8:30 AM to 4:30 PM, Monday through Friday, including a 30-minute lunch break. Further communication regarding this position will be conducted via email.

QUALIFICATIONS:
Minimum Experience and Training Requirements:


Applicants must meet one of the following criteria:
1. Six months of experience as a County Caseworker 1;
2. Successful completion of the County Social Casework Intern program;
3. A bachelor's degree in social welfare or a related field with 12 college credits in relevant social sciences and one year of professional social casework experience;
4. Any equivalent combination of experience and training that includes the required educational credits.

Individuals nearing graduation with a qualifying degree or credits will be considered to meet the educational requirements.


OTHER REQUIREMENTS:
Currently, the PA residency requirement is waived for this position. Candidates must be capable of performing essential job functions.

LEGAL REQUIREMENTS:
Successful candidates must pass a background investigation, which includes obtaining satisfactory criminal history reports as mandated by the Child Protective Services Law.
