ROLE DESCRIPTION

The Case Coordinator plays a vital role in the delivery of services, focusing on the essential tasks of identifying, coordinating, and overseeing necessary services and supports for participants enrolled in waiver programs.

Identifying services involves assisting participants and their families in connecting with, arranging, and acquiring services outlined in an Individual Support Plan (ISP), which may include medical, social, educational, or other community resources.

Coordination entails the creation and continuous management of the ISP in collaboration with the participant, their family, ISP team members, and service providers.

Oversight includes maintaining regular contact with participants and their families to ensure that services are delivered according to the participant's established plan.

In addition to these core functions, Supports Coordination also encompasses providing guidance and support to help participants decide on the direction of their services, including assistance for those who choose to direct their own services.

WORK SCHEDULE AND ADDITIONAL INFORMATION:
  • Full-time position.
  • Standard working hours are from 8:30 AM to 4:30 PM, Monday through Friday, with a 30-minute lunch break.

QUALIFICATIONS AND REQUIREMENTS

Minimum Experience and Training:

  • At least six months of experience as a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • A bachelor's degree in social welfare; or
  • A bachelor's degree supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences, along with one year of professional social casework experience in a social service agency; or
  • Any equivalent combination of experience and training that includes 12 college credits in relevant fields.
  • Applicants nearing graduation with a qualifying degree/credits will be considered to meet educational requirements.
Other Requirements:
  • Residency requirement is currently waived for this position.
  • Ability to perform essential job functions is necessary.

  • Successful completion of a background investigation is mandatory.
  • This position is subject to the Child Protective Services Law, requiring satisfactory criminal history reports.
