ROLE DESCRIPTION
The Caseworker/Supports Coordinator plays a vital role in delivering services that encompass the essential tasks of identifying, coordinating, and overseeing necessary services and supports for waiver participants.
Key Responsibilities:
1. **Identifying Services:** Assist participants and their families in connecting with, arranging, and acquiring services outlined in an Individual Support Plan (ISP), which may include medical, social, educational, or other community services.
2. **Coordination of Services:** Develop and manage the ISP in collaboration with the participant, their family, the ISP team, and service providers.
3. **Oversight of Services:** Maintain regular contact with participants and their families to ensure that services are delivered according to the participant's plan.
Additionally, Supports Coordination includes providing guidance and support to help participants decide on service direction and assistance for those who choose to direct their own services.
WORK SCHEDULE AND ADDITIONAL INFORMATION:
This is a full-time position with standard working hours from 8:30 AM to 4:30 PM, Monday through Friday, including a 30-minute lunch break. Further communication regarding this position will be conducted via email. Please ensure to check your email for updates.
QUALIFICATIONS AND REQUIREMENTS:
**Minimum Experience and Training Requirements:**
- Six months of experience as a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- A bachelor's degree in social welfare; or
- A bachelor's degree supplemented by 12 college credits in relevant social sciences and one year of professional social casework experience in a social service agency; or
- Any equivalent combination of experience and training including 12 college credits in relevant social sciences.
Applicants nearing graduation with a qualifying degree/credits will be considered to meet the educational requirements.
OTHER REQUIREMENTS:
Currently, the PA residency requirement is waived for this position. Candidates must be capable of performing essential job functions.
LEGAL REQUIREMENTS:
Successful candidates must pass a background investigation. This position is subject to the Child Protective Services Law, requiring satisfactory criminal history reports, including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
