POSITION OVERVIEW

The Client Services Coordinator plays a crucial role in facilitating the daily intake and referral processes to ensure clients receive appropriate services. This position is responsible for coordinating in-home care requirements while adhering to Federal and State regulations, as well as Managed Care Organization (MCO) guidelines. The coordinator manages a caseload of clients enrolled in Medicaid programs and conducts regular assessments to adjust care plans as necessary.

KEY RESPONSIBILITIES

  • Oversee and manage a caseload of clients enrolled in Medicaid programs.
  • Ensure coordination of clients' in-home care needs in alignment with Federal and State regulations, MCO program guidelines, and contractual obligations.
  • Facilitate daily intake and referral processes to support business growth and development.
  • Foster a collaborative and supportive office environment.
  • Assist in daily operations and advocate for clients' needs.
  • Work independently to meet position requirements with minimal supervision.
  • Engage with stakeholders to enhance service delivery.

QUALIFICATIONS

  • A Bachelor's degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology, or a related field is preferred.
  • At least one year of experience working with individuals with disabilities or the elderly is preferred.
  • A combination of education and relevant experience will be considered.

The role typically involves working in an office setting, utilizing a computer, telephone, and other office equipment to fulfill responsibilities. The work environment is generally consistent with that of an office, with potential for frequent interruptions. The incumbent is regularly required to sit, communicate, and hear; frequently engage in repetitive hand motions, and may need to stand, walk, reach, bend, or lift items weighing up to 20 pounds. Reasonable accommodations may be provided to enable individuals with disabilities to perform essential functions.
