Job Preview at a Glance: The Case Manager serves as a central support within the interdisciplinary team. This staff member conducts assessments, assists members with resources within the community and provides coordination of care inside and outside of the agency. Responsibilities:
  • Intake screenings (Completing intake paperwork and referrals)
  • Completing annual behavioral health assessments, including service planning and support planning
  • Facilitating CFTs and ARTs
  • Facilitating groups
  • Identifying gaps in care for our members
  • Completing SMI and SED evaluation
Qualifications:
  • Minimum of 21-year of age
  • Bachelor's degree in the behavioral health field plus one year of relevant experience in behavioral health
  • A combination of experience and education totaling 2 years is acceptable.
  • Valid AZ driver's license & motor vehicle clearance.
  • Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire.
