Company Overview

Nation-wide organization specializing in geriatric primary care is seeking a dedicated and compassionate Primary Care Nurse Practitioner (NP) to join their team. Job Summary

This role involves providing high-quality patient care to the geriatric population in a collaborative care setting. The ideal candidate will have experience working in primary care or skilled nursing facilities/nursing homes and possess strong communication skills.

Key Responsibilities:
  • Monday-Friday schedule, 8am-5pm, with no nights or weekends
  • 12-16 patients per day once fully ramped up
  • Collaborative care team consisting of an MD, APP, Scribe, MA, and Shared RN/Social Work support
  • Specialty support from partnership with outside company
  • Innovative EMR system created internally, with training provided
Requirements
  • Board Certification Required
  • CT State License Required
  • Training as a Family or Adult NP preferred
  • Previous primary care or skilled nursing facility/nursing home experience preferred
Estimated Salary: $120,000 - $150,000 per year

This salary estimate is based on industry standards and takes into account the location, job requirements, and level of experience. The actual salary may vary depending on individual qualifications and negotiations.
