About the Job
We are seeking an experienced APRN to join our team of dedicated healthcare professionals. As a Primary Care Nurse Practitioner, you will be responsible for providing exceptional patient care in a dynamic primary care setting, aligned with our mission and policies.
Duties and Responsibilities
- Deliver high-quality primary medical care, utilizing your expertise and skills to diagnose and treat patients.
- Develop personalized plans of care, incorporating medical history, physical examination, diagnosis, and appropriate treatment or referral as needed.
- Utilize various diagnostic tools and resources, such as laboratory tests, in a cost-efficient manner to inform treatment decisions.
- Maintain open communication with colleagues, patient families, and support staff to ensure seamless care coordination.
- Refer patients requiring specialist services while maintaining primary care responsibility to ensure continuity of care.
- Prominently maintain accurate medical records within our electronic health record system and perform necessary charting and follow-up actions.
- Participate in Medical & General staff meetings and relevant clinical teams to stay informed and contribute to quality improvement initiatives.
- Pursue ongoing education and training to enhance medical knowledge and provide top-notch patient care.
- Perform other essential duties as required by Fair Haven Community Health Care to achieve our goal of delivering exceptional service and healthcare.
Qualifications and Requirements
- Graduation from an accredited APRN program.
- Licensure to practice as an APRN in the State of Connecticut.
- Basic Life Support certification.
- Bilingual proficiency in Spanish and English is highly desirable.
