Job Title: Triage Registered Nurse

The Triage Registered Nurse plays a vital role in our team, reporting directly to the Associate Clinical Director. This position is responsible for triaging patient and caregiver telephone calls, providing solutions, palliating symptoms, and offering education. The successful candidate will assist in identifying patient/family care needs and provide nursing and supportive care for terminally ill patients and their families, in conjunction with case managers and physicians.

Key Responsibilities:

  • Triage all patient and caregiver calls, providing solutions and palliation of symptoms
  • Serve as a liaison with physician offices on behalf of the care team
  • Provide immediate response and emotional support to families at the time of death calls
  • Assist in coordinating changes in patient level of care
  • Communicate with insurance case managers on behalf of the care team, documenting discussions in detail
  • Assist with pharmacy issues, ordering medication refills, securing scripts for controlled substances, and problem-solving breakdowns in systems communications
  • Order supplies and durable medical equipment as necessary, documenting in the EMR
  • Coordinate equipment pickup with vendor representatives as needed
  • Trend calls prospectively to identify patterns by team/provider and opportunities for improvement
  • Deploy staff as needed to address patient and family-assessed needs

Requirements:

  • Diploma, ADN, and/or BSN from an accredited institution
  • Current Maryland State license as a Registered Nurse
  • One to two years of nursing experience, preferably in Home Health Hospice or triage
  • Strong time management and organizational skills
  • Excellent interpersonal and communication skills, with the ability to interact with a diverse population
  • Ability to be patient and family-focused
  • Knowledge of universal precautions for infection control and safe nursing standards
  • Ability to perform consistently in stressful situations
  • Strong and creative problem-solving skills, with the ability to prioritize and delegate
