Position Overview:
The Clinical Nurse Coordinator for Cardiovascular plays a pivotal role in enhancing patient care through effective collaboration and management.
Key Responsibilities:
- Collaborates with healthcare providers to manage patient inquiries, conduct triage assessments, oversee clinical documentation, and coordinate electronic medical advice communications.
- Partners with providers to conduct Medicare Annual Wellness Visits, aiming for a target of six visits weekly.
- Assists the practice manager in pre-visit planning, ensuring that necessary tasks are completed to address care gaps and meet quality standards.
- Acts as a clinical resource and educator, fostering team development in conjunction with providers, practice managers, clinical leads, and care managers.
- Reviews quality metrics with the Medical Group Director, Program Consultant, and Director of Community and Population Health to ensure optimal care delivery.
Qualifications:
- Bachelor's degree preferred (or currently pursuing).
- A minimum of three years of clinical nursing experience.
- At least one year of experience in care coordination or familiarity with primary care practice dynamics.
- Must possess a current Maryland Nursing License.
- Care Management Certification is preferred.
- BLS certification is required.
