Job Summary:

The Home Health Intake Coordinator is responsible for efficiently processing incoming referrals for all branches, ensuring compliance with regulations and requirements for admission to Home Health. This role requires effective communication with physicians, patients, and sales and marketing staff, as well as coordination and confirmation of Face to Face visits and attestation forms.

Key Responsibilities:

  • Process incoming referrals for all branches
  • Understand and comply with regulations and requirements for admission to Home Health
  • Communicate with physicians, patients, and sales and marketing staff
  • Coordinate and confirm Face to Face visits and attestation forms
  • Promote compliance with incoming documentation
  • Monitor current admission medicare to private insurance percentages
  • Run and maintain intake reports within HCHB
  • Maintain a referral tracking sheet

Qualifications:

  • Current LPN licensure in the state of Indiana
  • Business and professional setting experience
  • Healthcare experience preferred
  • Excellent written and verbal communication skills
  • Ability to speak, read, write, and comprehend English
  • Ability to handle multiple tasks simultaneously and meet deadlines
  • Excellent organizational and time management skills
